How Does a Chemical Peel Work?

Chemical peels have been used for centuries to help rejuvenate skin and reduce the signs of ageing. Peels cause a controlled injury to the skin, triggering the skin’s healing process, resulting in rejuvenated skin. They can also remove abnormal collagen structures, meaning they can significant improve the appearance of scars.

The results you achieve from peels can be very dramatic and are directly related to the depth of the peel. Peels are generally grouped into superficial, medium depth and deep peels.

  • Superficial peels: only affect the the epidermis.
  • Medium-depth peels: extend to the papillary (superficial) dermis.1Lee KC, Wambier CG, Soon SL, et al. Basic chemical peeling: Superficial and medium-depth peels. J Am Acad Dermatol. 2019;81(2):313-324. doi: 10.1016/j.jaad.2018.10.079
  • Deep peels: penetrate to the mid-reticular dermis.2Wambier CG, Lee KC, Soon SL, et al. Advanced chemical peels: Phenol-croton oil peel. J Am Acad Dermatol. 2019;81(2):327-336. doi: 10.1016/j.jaad.2018.11.060

What can be treated with a Chemical Peel?

The conditions that can be treated with a face peel depend on the depth of penetration in the skin:

Superficial Chemical Peel

Superficial peels are effective for mild acne, melasma and rejuvenation. A common brand used for melasma is the Cosmelan peel.

Common peels include 20-30% salicylic acid, 10-20% trichloroacetic acid (TCA) and Jessner solution.

A salicylic acid skin peel is particularly effective for acne, reducing it by 43% on average with a single treatment.3Dainichi T, Amano S, Matsunaga Y, et al. Chemical peeling by SA-PEG remodels photo-damaged skin: suppressing p53 expression and normalizing keratinocyte differentiation. Medium-Depth Chemical Peel

Medium-depth peels are an effective face peel for lines, wrinkles, texture, superficial scars and rejuvenation. Medium-depth peels should not be used outside of the face or scalp.

Common medium-depth face peel ingredients include dry ice (solid CO2) + 35% TCA and Jessner + 35% TCA.

Deep Phenol Face Peel

Deep peels provide the most dramatic results when used as a face peel.

A croton oil-phenol face peel can treat severe wrinkles and acne scars. It can even be used instead of filler to augment the lips.

Due to the concern of heart toxicity with deep peels, they are not often performed. We don’t currently offer phenol peels at Anue.

Preparing for a Skin Peel

It is important that you keep the treatment area out of direct sunlight and avoid tanning for four weeks prior to the treatment session. While tanned skin can be treated, we will need to modify the settings, compromising your results and it will increase the risk of adverse outcomes.

Post treatment care for a Chemical Peel

The full effects of treatment can take 4-12 weeks to become visible. Following treatment, it is normal to experience some discomfort, redness, swelling, and a sensation like a mild sunburn. These effects typically subside within a few hours to days depending on the treatment method. Some flaking of the skin for a few days is also normal.

We recommend a follow-up appointment four weeks after treatment to assess your results, individual response, and to plan additional treatments if required.

Immediately after treatment

  • Avoid touching or putting pressure on the treated area for at least 6 to 8 hours.
  • Apply a cold compress or ice pack to reduce any discomfort or swelling.
  • Minimise sunlight exposure and use sunscreen.
  • Treated areas may initially darken, then lighten after 48 to 72 hours.

Skincare

  • Use a soap-free cleanser and a non-fragrance moisturiser.
  • Avoid exfoliation and any strong skincare products around the treated area for at least 3 days. This includes retinol (vitamin A) and vitamin C.
  • Apply sunscreen with an SPF50 or higher to protect the treated area and avoid prolonged sun exposure for at least two weeks post-treatment.

Activities

Avoid strenuous exercise, hot showers, saunas, and steam rooms for at least 24-48 hours post-treatment. Also, avoid swimming or soaking in hot tubs for at least 48 hours.
